Overview

The 2021 “Behavioral and Psychological Treatments for Chronic Insomnia Disorder in Adults: an American Academy of Sleep Medicine Clinical Practice Guideline” strongly recommends cognitive behavioral therapy for insomnia (CBT-I) as the first-line treatment for chronic insomnia. However, numerous barriers impede CBT-I’s reach and uptake in routine care. A significant obstacle limiting the application and effectiveness of real-world practice is the lack of adaptation and testing of CBT-I for diverse populations with complex needs, such as adolescents, adults with substance use disorders, night shift workers, and economically marginalized adults. It is essential to adapt CBT-I to address the unique challenges faced by these underrepresented groups and overcome barriers to successful implementation in practice.

This session provides examples of recent efforts from AASM Foundation-funded projects to utilize implementation science frameworks and theories in adapting and implementing CBT-I among novel populations. These adaptations can potentially enhance the acceptability and appropriateness of CBT-I and optimize treatment “fit” across care contexts.
